Depends entirely on the community. Legally the definition is left up to community standards and what is considered obscene a small town in Amish Pennsylvania probably wouldn't be considered obscene in Los Angeles, California.

They might not see it but the FBI will. That's how Rob Black (Robert Zicari) was busted for obscenity. The feds ordered his extreme videos from a conservative community in Pennsylvania and when he shipped them, they busted him. That case is still going on.
